Referral Process

The admissions process begins with a phone call to Hospice made by the patient, a family member, friend, physician or other healthcare professional. One of our Admission Coordinators will ask for general information including permission to contact the patient's personal physician to make sure the patient is eligible for hospice care. A time will be arranged for a Hospice staff member to visit with the patient and/or family to explain how we can help. If the patient is eligible and our service is requested, we will take care of the rest.

Hospice Eligibility

When a patient meets medical criteria and their physician certifies qualification, they can receive hospice services.
